
OCI-R Scoring and Interpretation

Scoring Procedure for the OCI-R
Each of the 18 items is scored 0 to 4, with a higher item response indicating greater distress from that obsessive-compulsive symptom over the past month. Item scores are summed to produce a single total score ranging from 0 to 72, and separately summed within each of the six 3-item subscales to produce a subscale score ranging from 0 to 12 (Foa et al., 2002). Reverse-Scored Items
None. All 18 items are scored in the same direction.
Subscale Computation
- Washing subscale: sum of items 5, 11, and 17.
- Checking subscale: sum of items 2, 8, and 14.
- Hoarding subscale: sum of items 1, 7, and 13.
- Ordering subscale: sum of items 3, 9, and 15.
- Obsessing subscale: sum of items 6, 12, and 18.
- Neutralizing subscale: sum of items 4, 10, and 16.
The full-scale total score is the sum of all 18 items (equivalently, the sum of all six subscale scores).
Cutoff Scores and Severity Bands for the OCI-R

Severity bands: Abramovitch et al. (2020), derived from a large clinical OCD sample benchmarked against the Yale-Brown Obsessive-Compulsive Scale, the clinician-administered diagnostic standard.
Separately, Foa et al. (2002) established diagnostic differentiation cutoffs in the original validation sample: a total score of 21 or higher was the optimal cutoff for distinguishing individuals with OCD from those with no psychiatric diagnosis, while a total score of 18 or higher was the optimal cutoff for distinguishing individuals with OCD from those with a different anxiety disorder. These two figures describe diagnostic differentiation between groups, not symptom-severity intensity, and are distinct from the Mild/Moderate/Severe bands above.
There are no widely published, separately validated severity bands for the six individual subscale scores; subscale scores are typically interpreted by comparing relative elevation across the six domains rather than against fixed cutoffs.
Interpreting Your Score on the OCI-R
A higher total score indicates greater overall obsessive-compulsive symptom severity. Because the OCI-R also produces six subscale scores, a respondent's pattern across Washing, Checking, Hoarding, Ordering, Obsessing, and Neutralizing can highlight which symptom domains are most elevated, even when the total score alone would not.
The OCI-R is a screening and severity-tracking tool, not a diagnostic instrument. A score in the Moderate or Severe range, or a total score above the Foa et al. (2002) diagnostic differentiation cutoffs, indicates elevated likelihood of clinically significant obsessive-compulsive symptoms, but does not on its own constitute a diagnosis. Interpretation should be made in the context of a full clinical evaluation by a qualified professional, typically using the clinician-administered Yale-Brown Obsessive-Compulsive Scale.
Reliability and Validity of the OCI-R (Extended)
Beyond the primary summary on the instrument page, further findings extend the evidence base for the OCI-R:
- Subscale reliability varies by sample and symptom type: Across the six subscales, internal consistency in a sample of 186 OCD patients ranged from α = .57 for Neutralizing (the lowest) to α = .93 for Hoarding (the highest) (Huppert et al., 2007). The Neutralizing subscale's comparatively weak and inconsistent reliability across studies likely reflects that its three items (numbers, counting, repeating) tap a less behaviorally consistent construct than more overt symptoms like washing or checking.
- Hoarding as a distinct construct: Splitting the OCI-R into a 15-item non-hoarding scale and a separate 3-item hoarding scale produced a better model fit than treating all six subscales as one structure, consistent with the DSM-5's reclassification of hoarding disorder as diagnostically separate from OCD (Wootton et al., 2015). Researchers screening specifically for OCD symptom severity should be aware that a total score combining all six subscales still includes hoarding-related content.
- Discriminant and clinical-group evidence: In the original validation study, the OCI-R total score and each subscale reliably distinguished OCD patients from both nonclinical individuals and from patients with a different anxiety disorder, supporting its use as a differential screening tool rather than only a general distress measure (Foa et al., 2002).
The OCI-R has also been independently translated and validated in numerous languages and populations, including Japanese (Koike et al., 2017) and Korean (Woo et al., 2010) versions.
