Generalized Anxiety Disorder 7-Item Scale (GAD-7)

The GAD-7 is a self-report instrument that measures the severity of generalized anxiety disorder (GAD) symptoms. It was developed by Spitzer, Kroenke, Williams, and Löwe (2006), in a study funded by an unrestricted educational grant from Pfizer. A 2-item ultra-brief version, the GAD-2, is also available for rapid screening (Kroenke et al., 2007), and the GAD-7 has been independently adapted and validated in other languages, including a Spanish-language version (García-Campayo et al., 2010).

Items and Structure of the GAD-7

The GAD-7 consists of 7 items, each describing an anxiety symptom experienced over the last 2 weeks (e.g., "Worrying too much about different things"), scored on a 4-point Likert scale: 0 ("Not at all"), 1 ("Several days"), 2 ("More than half the days"), or 3 ("Nearly every day"). Spitzer and colleagues describe the GAD-7 as brief and efficient, noting it can be completed entirely by the patient, though the original paper does not report a formal timed completion metric; a Spanish-adaptation validation study measured average completion time at 2 minutes 30 seconds (García-Campayo et al., 2010).

The 7 items were selected from an initial 13-item pool that reflected DSM-IV diagnostic criteria for GAD and existing anxiety scales, retained for having the highest correlation with the full 13-item total score (r = 0.75-0.85). The first 3 items capture the two core DSM-IV diagnostic criteria for GAD (Spitzer et al., 2006).

Scoring Summary

The 7 item scores are summed into a single total score ranging from 0 to 21, with higher scores indicating greater anxiety symptom severity. There are no subscales.

Psychometric Properties of the GAD-7

  • Reliability: In the original validation sample, the GAD-7 showed excellent internal consistency (Cronbach's α = .92) and good test-retest reliability (intraclass correlation = 0.83) over a 1-week interval (Spitzer et al., 2006).
  • Validity: Convergent validity was supported by correlations with the Beck Anxiety Inventory (r = .72) and the anxiety subscale of the Symptom Checklist-90 (r = .74). Factor analysis of the GAD-7 alongside an 8-item depression measure confirmed anxiety and depression as empirically distinct dimensions: anxiety items loaded 0.69-0.81 on one factor, while depression items loaded 0.58-0.75 on a separate factor, together explaining 63% of total variance (Spitzer et al., 2006).
  • Normative Information: Severity bands (see the Scoring and Interpretation guide) were derived from the original US primary care validation sample (Spitzer et al., 2006). In a separate large German general population sample (N = 5,030), mean GAD-7 scores were 3.2 (SD = 3.5) for women and 2.7 (SD = 3.2) for men, with approximately 5% of the sample scoring 10 or greater and 1% scoring 15 or greater, providing percentile-based population norms (Löwe et al., 2008).

Recommended Use and Applications of the GAD-7

The GAD-7 is used across clinical, research, and organizational settings to screen for and quantify generalized anxiety symptom severity.

  • Mental health screening: Used in primary care and general population settings as a brief screening measure for probable generalized anxiety disorder, ahead of a full diagnostic evaluation (Spitzer et al., 2006; Löwe et al., 2008).
  • Treatment-outcome and longitudinal monitoring: Administered repeatedly to track anxiety symptom change over the course of a psychological intervention, with a minimal clinically important difference of 4 points on the total score (Toussaint et al., 2020).
  • Occupational and student wellbeing research: Used in university and athletic-department studies examining anxiety, functional impairment, and mental health in non-clinical populations (Tran, 2020).
  • General psychiatric and clinical research: Used to assess anxiety severity across heterogeneous psychiatric samples, alongside other measures of psychological distress (Johnson et al., 2019).
