GAD-7 Scoring and Interpretation

Scoring Procedure for the GAD-7

Each of the 7 items is scored 0 to 3, with a higher item response indicating greater anxiety symptom frequency over the last 2 weeks. Item scores are simply summed to produce a single total score ranging from 0 to 21, with no subscales (Spitzer et al., 2006). Cutoff scheme: Spitzer et al. (2006). A score of 10 or greater is also used as a separate diagnostic threshold for identifying probable cases of GAD, balancing sensitivity (89%) and specificity (82%) in the original validation sample; this threshold is distinct from the severity bands above, which describe symptom intensity rather than diagnostic likelihood.

Interpreting Your Score on the GAD-7

A higher total score indicates greater anxiety symptom severity over the past 2 weeks. Because the GAD-7 produces a single total score, there is no separate subscale interpretation to consider.

The GAD-7 is a screening tool, not a diagnostic instrument. A score in the Moderate or Severe range, or a total score of 10 or greater, indicates elevated likelihood of generalized anxiety disorder, but does not on its own constitute a diagnosis. Interpretation should be made in the context of a full clinical evaluation by a qualified professional.

Reliability and Validity of the GAD-7 (Extended)

Beyond the primary summary on the instrument page, further findings extend the evidence base for the GAD-7:

  • Independent psychiatric-sample replication: In a heterogeneous psychiatric sample, the GAD-7 showed good internal consistency (α = .88 overall, ranging .83-.93 across clinical units). A confirmatory factor analysis required allowing correlated residuals among items 4-6 (trouble relaxing, restlessness, irritability, the more somatic items) to achieve good model fit, and convergent validity was supported by correlations with the Beck Anxiety Inventory (r = .69) and the Beck Depression Inventory (r = .58) (Johnson et al., 2019).
  • Sensitivity to change: In a longitudinal study of a chronic-depression outpatient sample, the GAD-7 showed significant score reductions over 12 and 48 weeks of treatment, and a minimal clinically important difference of 4 points on the total score was established (Toussaint et al., 2020). That same paper reviews similar sensitivity-to-change findings reported elsewhere in acute psychiatric samples and large routine-care cohorts from the English Improving Access to Psychological Therapies (IAPT) services.

The GAD-7 has also been independently translated and validated in numerous languages and populations beyond the original US primary care sample. Reliability and factor-structure findings vary by translation and population; consult the specific translation's validation paper for population-specific figures.
